Documents Required for Enrolment at OHS
Documentation for IN ZONE applications
1. Statutory Declaration:
Onehunga High School Statutory Declaration completed and signed by a Solicitor or Justice of the Peace.
2. NZ citizenship
2a. An original or certified NZ / Australian birth certificate.
OR
2b. NZ & Australian copy of New Zealand or Australian passport (photo page) .
OR
2c. Other Citizens: Certified copies of passport (photo page) and current visa together with parents' passports and residency or work visas.
3. Two forms of proof of residential address
3a. One document showing the parent’s/caregiver’s name and address, of where they currently live: Such as power electricity, gas, water, phone or internet bill.
AND
3b. Rates account OR tenancy agreement.
4. If the student lives with a guardian (not a parent or court-appointed legal guardian), this form must be completed.
5. A full copy of your child's most recent school report.
6. Health information (as provided in the online enrolment form) and immunisation record.
Documentation for OUT OF ZONE applications
For out of zone enrolments follow the IN-ZONE application process, attaching ALL of the the documents requested. Apart from:
You do not need to provide a Onehunga High School Statutory Declaration.
You DO need to provide ONE of below, depending on your circumstance:
- Sibling is a current student - sibling's full birth certificate.
- Sibling is a past student - sibling's full birth certificate
- Parent is a past student - Parent's passport photo page
